Anamnesis

Niú**, male, age 6. At around 1.5 years, developmental regression occurred: speech and cognitive abilities dropped to zero, no eye contact. From the year before last, received IVIG 10 administrations + pulse methylprednisolone 3 times with improvement — speech and cognition recently emerged, but problems persist and condition remains unstable. CSF and serum negative for autoimmune antibodies, but myelin basic protein was elevated and CSF pressure also high; lymphocyte subset disturbance. Treated as inpatient in Shanghai Children's Hospital Department of Neurology with diagnosis of immune-mediated encephalitis; episodes of abnormal EEG activity persist. These years affected child's development, lagging and slow. Currently receiving weekly murine NGF injections 20 µg and morning/evening oxcarbazepine. Due to unstable condition, sometimes overly excited with many abnormal movements and self-talk.

2022-03-21 — intravenous placental mesenchymal stem cells (P-MSC) 2U, every 2 weeks, 4 administrations = 1 cycle.

Outcome

4 administrations total:

  • 1st: 10 days post-infusion, child's condition visibly good, more focused, quickly enters learning mode.
  • 2nd: post-infusion, child no longer makes pig-grunting sounds (from "Peppa Pig"); can choose and remember songs — recalls about 10 song lyrics, memory markedly improved, firm pen grip, good learning and mental state.
  • 3rd: marked improvement in motor activity, abnormal movements significantly reduced; hands still show unconscious stereotypies; remembers more children's songs and sings along.
  • 4th: after returning home, learned to ride a bicycle in a short time.

Currently child's mental state is excellent, movement improved, memory, language, and cognition show marked improvement.

Summary of the child's problems. At around 1.5 years — developmental regression to zero speech and cognition, no eye contact. From the year before last — medication 10 times + pulse methylprednisolone 3 times with improvement: speech and cognition emerged, but condition unstable. CSF negative for autoimmune antibodies, but CSF pressure elevated, lymphocytes elevated. Subsequently treated as inpatient in children's hospital pediatric neurology for immune-mediated encephalitis; episodes of abnormal activity persist. Medication in recent years has helped — child more active and stable. Currently weekly IVIG 200g, morning/evening oxcarbazepine. Due to unstable condition, painful injections, many abnormal movements, self-talk.

Child currently weighs 18.5 kg, just turned 6 years old.

Physician conclusion

No adverse reactions observed. Stem cell transplantation for pediatric autism effectively alleviates associated symptoms.
